Health insurance coverage is an important concern in any medical situation. Foothills Family Practice works with most health plans and is “in-network” for thousands of people. However, if you live outside the state of Colorado these same health plans may determine services at Foothills Urogynecology to be “out of network”. Contact the Customer Service Department at your health insurance company to find out the details of your policy. They should be able to tell you about your coverage and financial responsibility for your care at Foothills Family Practice. If you are coming from outside Colorado, please make sure you know your benefits and authorization requirements for non-emergent out patient hospital services outside your home state. Foothills Family Practice is an ambulatory physician practice and the providers are employees of the practice. You will most likely be required to pay a primary (non-specialty) physician co-pay. Other co-payments may apply; deductible or co-insurance. You’ll also be responsible to pay additional co-payments, deductibles, co-insurance or fees associated with third party services that we refer you to, such as laboratory services, radiology and other diagnostic services – as well as any additional costs associated with other services. To find out if we are contracted with your specific insurance company and are in-network, please contact your Insurance Company’s Member Services Department or visit their website.

• It is the patient’s responsibility to be generally informed about their insurance coverage, including (but not limited to) benefits, deductibles, referrals, authorizations, co-payment, coverage/coverage limitations, patient responsibility, etc.
• It the patient’s responsibility to make payment arrangements for service rendered by Foothills Family Practice and other third party facilities (for things like laboratory services, radiology, diagnostic services, etc.)
• Foothills Family Practice can assist you in working with your health insurance provider to obtain authorization or approval to be seen/receive services at Foothills Family Practice. It is however, the patient’s responsibility to obtain such authorization and to ensure that such authorization has been attained prior to services being rendered.
• It is the responsibility of self-pay patients to follow and understand Our Policies and, if applicable, Self-Pay patient policies.
Once you have an appointment to be seen at Foothills Family Practice, our team can work with you, your physician and your health plan about issues related to insurance and billing
