Your Initial Appointment & What to expectWhen you call to schedule your initial appointment, a Scheduler will collect pertinent demographic information from you, such as your name, address, phone number, date of birth and insurance information. We will then work with you to find the soonest available new patient appointment that accommodates your schedule. If we determine, based on the insurance information you provide to us, that a referral or authorization will be required for this visit, we’ll discuss the process of obtaining it with you ― and explain what your responsibilities may be in that process. Once we’ve scheduled your appointment we’ll offer to either ask you to complete New Patient Packet the day of your appointment (please arrive 30 minutes early), or ask you to download and print it from our website (you’ll need to complete it and bring it with you at the time of your appointment). You should expect a full physical examination; please be prepared to provide a urine or blood samples.
You’ll need to bring the following items to your appointment: • Government Issued Photo ID (i.e. Drivers Licensee) • Most Recent Insurance Cards for your primary plan (and secondary, if applicable) • Completed New Patient Packet (demographics and medical history questionnaire) AND • Any list of any questions or concerns that you would like to discuss with the provider. • A current list of all of the medications (including Over-the-Counter), vitamins and supplements you take. • A list of all allergies you suffer from (environmental and pharmaceutical) and associated reactions.
