Prior to your appointment at Foothills Family Practice, please download and complete the following forms:

Transfers: Some patients may need to transfer their records either to or from another medical practice. If you need to transfer your medical records to or from another medical office, please complete the following form and fax it to the medical office where the records currently reside. If your records are currently stored at Foothills Family Practice, use fax number 303-986-2205.
