Ultimately, it is the responsibility of the patient to make sure that we are compensated for all medical services. In order to support our patients, we will submit one proper medical insurance claim per visit. Please bring your insurance card or paper work with you on every visit. Be sure to complete all forms to the best of your ability. If a proper insurance claim is submitted based on your information and payment is denied, you will be responsible for paying all charges. We cannot re-bill insurance carriers as a result of patients providing us with incomplete or incorrect information.
