All patients endergoing a complete routine physical examination are requested to fill out the following form in order to assist your provider:

FEMALE PHYSICAL EXAMINATION FORM
Patients looking to have their medical records transferred either to Foothills Family Practive from another office or from Foothills Family Practice to another providers office are asked to fill out this form and fax it to the office where the records currently reside.
